The below named client (hereinafter referred to as Client) hereby consent to, and authorize Danielle Morgese, a licensed Aesthetics practitioner, doing business as BEAUTY DEFINED, Inc., to perform the following treatment:

Pre-Care Instructions:

 

  • Please arrive without make-up and a clean face.

  • Do not consume Caffeine, Alcohol, Aspirin, Niacin, Vitamin E Advil/Ibuprofen or Power Drinks 24 hours before procedure.

  • Discontinue Glycolics, Chemical Peels and Retin-A 4 weeks prior. 

  • Refrain from use of any Alpha Hydroxy Acid (AHA) products close to the eyebrow area 2 weeks prior to and 2 weeks after your procedure. Check your moisturizer, facewash BB or CC creams and makeup primers for anything that says acid.

  • No brow waxing or tinting one week before and six to eight weeks post procedure.

  • You have to be off any kind of Accutane for 6 months. NO Exceptions!

  • No lash growth or brow growth serum for minimum of one month prior to permanent eyeliner or brow. 

  • For permanent eyeliner, eyelash extensions must be removed 3-5 day prior.

  • Fillers should be done six weeks prior to scheduled procedure or six weeks after procedure.

  • Botox should be performed two weeks prior to scheduled procedure or two weeks after.

  • Bring extra contact lens or eyewear for eyeliner procedures.

 

Cold Sores and Fever Blisters:

In the case of permanent lip color, cold sores and fever blisters MUST be treated. If you get cold sores or have ever had one in the past, you will need an antiviral prescription from your doctor before a lip procedure. Physicians usually instruct to take it 2 days before. As this procedure can bring out the virus if not medicated beforehand.

No lip filler 4-6 weeks before or after lip procedure.

 

Post Procedure: 

·       Do not touch the treated area unless applying ointment. No scratching, rubbing, or picking of the treated area should a scab should form. 

·       Do not apply makeup on treated area until the area has healed (about 2 weeks).

·       Follow with ointment for one week after treatment for brows and lips

·       Artificial tears may be used if needed but not recommended for after Eyeliner procedure.

I have read and understand the above information and of my own free will I choose to move forward with my procedure.

All my questions have been answered to my satisfaction and I consent to the terms of this Agreement. I do not hold the practitioner  responsible for any of my conditions that were present, or not disclosed at the time of this procedure, which may be affected by the treatment performed today. I also release and hold harmlzess the above-named practitioner and BEAUTY DEFINED, Inc., of any and all liability that may arise from the treatment(s) performed under this Agreement.
